Targeted Case Management

 

Children's Services

Targeted Case Management Services provide support to children and their families, and assist with making connections to needed resources and providers. The child and their family will work with a Case Manager to develop an individualized support plan that is specific to the child's needs. Pillars is able to offer services to children who fall into one or more of the following categories;

  • Children with Behavioral Health disorders

  • Children with Developmental Disabiliites

  • Children with Chronic Medical Conditions

Services are provided by MHRT-C staff with a Bachelor's Degree, who will assist the child and family with coordination of needed services, make referrals to additional resources as necessary, help with self-advocacy, and offer crisis intervention as needed.

Services for Adults

Targeted Case Management Services provide support during your integration into your community, and help you to connect with resources that you need. You will work with a Case Manager to develop an individualized support plan that is specific to your needs. Your Case Manager will help you with accessing resources such as;

  • Mental or behavioral health services and programs

  • Public transportation

  • Medical, dental, and vision care

  • Housing assistance

  • Fuel assistance

  • Food pantries

  • Vocational services

Services are provided by MHRT-C staff, who will help you advocate for yourself, coordinate referrals for services, and offer crisis intervention as needed. Pillars is able to offer services to adults who are experiencing homelessness, and adults who have received a diagnosis of HIV or AIDS.
