Community Support Services

 

Daily Living Support Services (DLSS)

DLSS services help you to maintain the highest level of independence possible. Services are provided in the comfort of your own home and in your surrounding community, in accordance with a support plan that you help design. DLSS services will assist you in remaining healthy and safe, and learning the skills necessary for daily living. These skills can include;

  • Symptom management

  • Personal hygiene

  • Maintaining housing

  • Managing medications and appointments

  • Organization skills

  • Meal planning and grocery shopping

Services are provided by MHRT-1 staff, who use support methods such as modeling, cuing and coaching.

Adult Case Management

Case Management Services provide support during your integration into your community, and help you to connect with resources that you need. You will work with a Community Integration Worker to develop an individualized support plan that is specific to your needs. Your CIW will help you with accessing resources such as;

  • Mental or behavioral health services and programs

  • Public transportation

  • Medical, dental, and vision care

  • Housing assistance

  • Fuel assistance

  • Food pantries

  • Vocational services

Services are provided by MHRT-C staff, who will help you advocate for yourself, coordinate referrals for services, and offer crisis intervention as needed. Case Management services are offered statewide.

Skills Development

Skills Development Services provides you with training and education in order for you to increase your independence and ability to use community resources. Goals can include, but are not limited to;

  • Budgeting and money management

  • Use of public transportation

  • Participation in social interests

  • Self-advocacy

  • Connecting with natural supports or support groups

Services are provided by a MHRT-C staff member, who will help you to design a service plan that is tailored to you and your needs.
