Nurse Care Manager

Position Summary:  The Nurse Care Manager (NCM) is a Behavioral Health Home (BHH) team member. The NCM works in a clinical oversight capacity to ensure that service plans are medically appropriate for each client, as well as coordinating periodic medical checkups for clients. The NCM is also responsible for tracking ER usage for each client.

Qualifications/Requirements:

  • Must be licensed in the state of Maine as one of the following; a Registered Nurse; a Psychiatric Nurse licensed as a Registered Professional Nurse and certified by the American Nurses Credentialing Center (ANCC) as a psychiatric and mental health nurse; a PMH-APRN who is licensed as a nurse practitioner or clinical nurse specialist and who has graduated from a child and adolescent or adult psychiatric and mental health nurse practitioner or clinical nurse specialist program and is certified by the appropriate national certifying body; or an Advanced Practice Nurse.

  • Prior experience in a BHH setting is preferred but not required

Essential Duties/Responsibilities

  • Provides primary care consultation and psychiatric care consultation

  • Coordinates with the BHHO, the primary care practice, and the client to provide BHH services, pursuant to the client's Individual Support Plan

  • Reviews Treatment Plans for clinical appropriateness and reevaluates treatment plans at appropriate intervals or as needed

  • Coordinates clients' use of Pillars programs and outside resources, including the use of any emergency room

  • Maintains accurate and timely clinical records

  • Participates in meetings, in-services, and supervisory sessions as required

  • Maintains current knowledge of best practices

  • Ability to serve a wide range of clients, including culturally diverse populations

  • Performs other job duties as assigned

Physical Demands:  While performing the duties of the job, the employee is regularly required to communicate in person or by telephone. The employee is frequently required to sit, stand, walk, reach, bend; use hands to handle or feel objects, tools or controls; and reach with hands and arms. The employee must occasionally lift and/or move up to forty (40) pounds.

Work Environment:  This position is performed in an office setting. The Nurse Care Manager is not generally expected to have face-to-face contact with clients.

Conditions of Employment.

  • Satisfactory reference and background investigation checks.

  • Demonstrated computer literacy through successful completion of pre-employment testing may be required.

  • Satisfactory completion of Child Protective Services background check.

  • Effective written and verbal communication skills

  • Familiarity with Electronic Health Records is preferred.

  • Completion of new employee orientation and ALL required paperwork prior to reporting for work.

  • Attendance to at all mandatory staff development and training.

  • Participation in payroll electronic deposit.

This is a full-time position. Payscale is $28.00 - $30.00 per hour.
