Behavioral Health Home Services
Pillars is a Behavioral Health Home Organization which has partnered with local Primary Care practices to offer Behavioral Health Home (BHH) services. BHH is a new model of care in which your case manager, a nurse, a clinical team leader, a medical consultant, and a peer support specialist work as a team, in partnership with your primary care doctor, to help you reach your goals for both mental and physical wellness.
BHH services are centered around an individualized plan of care that includes your strengths, needs, and goals for both your physical and mental health. Your Health Home Coordinator (Case Manager) can help you and your doctor work together, assist you with accessing social services and other supports, and help you get information about your care.
In addition to BHH services, Pillars will continue to provide Case Management services. Clients who are eligible for both services may choose which to use. If you are currently receiving Case Management services through Pillars, switching to BHH services is optional, and is entirely your choice.
